This form must be completed no less than two weeks prior to your event. 

Reminders:

  1. The Event Center hours of operation are 7:00am-10:00pm, use of the facilities before and after these times will result in a $100.00 per hour charge.
  2. The Event Center will provide equipment requested. It is the responsibility of the event (your group) to do all setup and take down of equipment unless other arrangements are made. There may be times when some equipment requested is not available.
  3. You are also responsible to make sure that the facility is left clean as found and equipment placed as found.
